When the print window comes up, the one with the list of printers and the Print
button to actually do the job, there's a line of tabs at the top I was missing.
(Da!) One of those tabs is "Options" and that's where you can tweak the headers
and footers. Set all to "none" and it sticks for all print jobs until changed
again. Life is good.
